The Maggot King is the repeatable solo boss added with The Blood Moon Rises on 30 June 2026. It lives in Vampyrium, the vampyre home realm the quest unlocks, and it is the source of the elder venator fang, the upgrade that turns a necklace of anguish into the necklace of rupture. For an ironman that is the reason to go: the best ranged neck slot in the game, gated behind nothing but the quest and a roughly 1/340 drop.
Every requirement, rate and mechanic below is taken from the OSRS Wiki as of 17 September 2026. The fight has been tweaked three times since launch (2, 8, 15 and 22 July), so older guides are already wrong about loot handling, clue drops and death fees.
Requirements
The boss itself needs nothing beyond the quest. The quest is the wall. Every skill requirement is unboostable.

The quest rewards are worth listing because they matter to an ironman before you ever fight the boss: 4 quest points, a tome of experience that grants 30,000 XP to any skill over level 70 and can be used six times (even on the same skill), the hallowed flail and sunspear, a fermenting vat in the Burgh de Rott hideout, and permanent vyre tolerance so Darkmeyer works without the noble outfit. Vampyrium itself adds bloodwood trees, seeking arrows, leechfin fishing, stymphike and letvek hunting, and venator slaying.
Why an ironman cares
The necklace of rupture is the best-in-slot ranged necklace: +20 ranged attack, +8 ranged strength and +3 prayer, which is +5, +3 and +1 over the anguish you already own. The upgrade material is untradeable, so this is the only way to get it. The crimson kisten is a strong crush special-attack weapon, and the resource table is dense with things an iron actually uses: noted uncut rubies, runite and adamantite ore, mort myre fungus, blood sacs, and stymphike feathers. The frequent dull ancient medal and stymphike tartare supply drop restores prayer, has a 3/7 chance to give combat stats, and heals, which often adds an extra kill per trip.
Hardcore Ironmen: the quest is not a safe death
Inside Vampyrium the quest respawns you at checkpoints with all your items kept, and the Drakan fights supply unlimited food and potions, so it feels like a minigame. The wiki is explicit that hardcore ironmen still lose their status if they die at any point during the quest. The Ver Sinhaza wave section is worse: deaths there are normal deaths, sending you to your respawn point with a gravestone to run back to. Bank a spare Drakan's medallion, over-prepare for every Drakan fight, and do not treat the free supplies as a safety net. The boss afterwards is a normal unsafe death too.
Getting there
Teleport with Drakan's medallion to Castle Drakan, enter Vampyrium and run east. Before the first kill, unlock the fast route: an aranei scout south-east of Sangvesti is under attack by three sanguidae. Kill all three, talk to the scout again, and it moves south of the bank chest next to the transportation icon. From then on it takes you straight to the forest east of Sangvesti, where the lair entrance sits among the darkwood trees near the Ruined Village.
Two agility shortcuts in the Sotfa Forest need 64 Agility. Use the one furthest from the scout: the closer one is a longer path to the boss and tends to run you into an ancient feral vyre. Either way expect the dire bat on the way to the scout; it is the only nuisance.
The Sangvesti bank chest has to be built before it works: 70 Construction (boostable), a hammer and saw, 1 magic stone, 2 cupronickel bars, 2 adamantite bars and 4 ironwood planks. If you would rather bank elsewhere, there is a shortcut south of Safalaan back to the scout.
Deaths at the boss respawn you in Vampyrium beside the portal, with your grave outside the lair entrance. Since the 15 July 2026 update the reclaim fee is cut by 75% until you have five kills, then the normal fee applies. The aranei scout by the portal will move your gravestone next to it for 50,000 coins, 1 vial of blood or 150 stymphike feathers, and since 8 July it can take the coins from Death's Coffer or your bank.
Mechanics
The Maggot King has 1,500 hitpoints, is fought alone in an instance, and cannot be attacked with melee until it falls below about 980 hitpoints (65%). You open at range with ranged or magic from your side of the arena, then finish with melee. It is weakest to crush and heavy ranged, has 200 Magic with +150 magic defence, and an 80% fire weakness. It is poisonous, so bring poison protection.
Its three basic attacks:
- Ranged (green spit) hits up to 46, is fully blocked by Protect from Missiles, leaves poisonous bile on your tile and two adjacent tiles, and poisons you starting at 8. Standing in bile does minor damage and drains 45 seconds of poison immunity. A serpentine helm plus anti-venom+ makes you immune to poisonous bile.
- Magic (yellow spit) hits up to 46, is fully blocked by Protect from Magic, and leaves sticky bile that drains Attack, Strength or Defence and delays your next attack by one tick. An inoculation bracelet makes you immune to sticky bile.
- Melee (head slam) is a 3x4 area attack for up to 55, reduced to about 35 by Protect from Melee, not blocked.
The rotation
It always opens with ranged. After 4 to 8 ranged or magic attacks it screeches (a white aura). Turn your protection prayer off before the screech or take 15 to 25 damage per tick, up to three times; turning it off late still cuts the total. During the screech its Defence is halved to 100, which is when a Zaryte crossbow special is most likely to land. Every screech also throws carrion onto random tiles: the initial piles hit for up to 25, the smaller splatter for up to 10, and both wipe any bile from the tiles they land on. Look for the isolated first shadows; the small bits only target tiles next to the first ones, so clustered shadows overlap onto tiles that look safe.
After the first screech its attack speed increases by one tick. It switches to magic until the next screech, then back to ranged, and so on. Any Defence you drained is restored on each screech, but defence bonuses stay, which is why the wiki rates the Eye of Ayak special above the accursed sceptre here.
Ur-maggot larvae
From about 1,250 hitpoints (83%) every screech also launches an ur-maggot larva at the arena. Shoot it with a standard ranged weapon (a bow with arrows) before it lands and it dies instantly; the shot fires immediately after your boss attack but still triggers your normal cooldown, so a faster bow is better. A larva that lands walks its row back and forth, eats carrion to heal the boss 1 hitpoint per pile, and if it reaches you it stops and chips at you to heal the boss, starting at 3 damage and rising by 1 each tick. The first larva comes from the west, then they alternate. From about 299 hitpoints (19%) it launches up to four at once, which is where a venator bow's bounce earns its slot.
The melee phase
From about 980 hitpoints (65%), after each screech it walks to the bone wall, lines its centre tile up with yours, and slams the 3x4 area in front of it for up to 55 (35 with Protect from Melee), spraying three bile pools within two tiles. Dodge by moving to its easternmost or westernmost tile. During this phase its magic defence jumps from +150 to +400, so mages should not expect much until it retreats.
The payoff is the melee punish: click the boss on the same tick it extends its head and you attack with no delay, with 85% of its defence roll removed if you are on crush, and with 50% more damage on that hit (multi-hit weapons only on the first hit). Slow, hard-hitting weapons benefit most, and punish damage does not care about attack speed. The wiki documents both a 4-tick cycle (step back to avoid losing ticks as it repositions) and a 5-tick cycle (move east or west with no tick loss), plus a full Dharok's method for experts who keep their hitpoints between 1 and 15 during melee phases. After a slam it screeches, retreats, and resumes whichever ranged or magic style it was using. From about 305 hitpoints (20%) it stops retreating and only melees, so the screech can be ignored from then on.
Gear for ironmen
Everything here is from the wiki's Maggot King strategy page, listed budget first. The boss is a hybrid fight: one style for the first 35% of its health, crush melee for the rest, and a bow for larvae.
Ranged (the ironman route)
Heavy ranged with enchanted ruby bolts is the cheap way in. Kills are longer than magic and the ruby self-damage means keeping your own hitpoints low, but nothing here is raid-locked.
- Weapon: rune crossbow → dragon crossbow or dragon hunter crossbow → Armadyl crossbow → Zaryte crossbow (its special doubles as the punish opener). A twisted bow is listed but sits below the heavy crossbows.
- Ammo: ruby bolts (e) → ruby dragon bolts (e). Seeking dragon arrows if you are on a bow.
- Larva bow: crystal bow, Bow of Faerdhinen, webweaver bow (nine to ten tile range) or a venator bow (bounces to the boss and to other larvae, slightly shorter range).
- Armour: elite void → eclipse moon → Masori (f). Eclipse moon reduces melee switches.
- Neck: amulet of fury → necklace of fangs → necklace of anguish → necklace of rupture.
- Cape: Ava's assembler → Dizana's quiver. The quiver carries both ammo types with no extra slots, but if you also bring a melee cape switch, remember to switch back or you will not fire.
- Off-hand: antler guard or odium ward → aquanite hopper → dragonfire ward → twisted buckler.
- Hands, feet, ring: void knight gloves or barrows gloves → Zaryte vambraces; aranea boots → avernic treads; venator ring.
Budget ironman ranged setup
Every item is from the wiki tier list. Bring a bow for larvae and a crush weapon for the melee phase.
Magic (fastest kills)
Fire spells add 5 stacking burn damage per hit and cut the boss's magic defence by 20 per stack, so the wiki puts fire surge with a tome of fire (sunfire runes, or fire runes with searing pages) above Tumeken's shadow, which gains none of that. Weapon order: purging staff, kodai wand, toxic staff of the dead or a volatile or eldritch Nightmare staff casting fire surge → Tumeken's shadow → twinflame staff casting fire wave → harmonised Nightmare staff. The harmonised staff and twinflame put out fire hits faster than an autocast staff, so burn stacks build and hold. Wear confliction gauntlets or a tormented bracelet, an elemental amulet or amulet of fire when casting fire spells (occult otherwise), an imbued god cape, Ahrim's or blue moon → virtus → ancestral, eternal boots or avernic treads, and a magus ring. Use the Eye of Ayak special once the boss has 5 burn stacks; a 30+ hit with it plus full burns leaves the boss with almost no magic defence, and the 50% cost regenerates within a kill.
Melee (from 65%)
Crush weapons get the accuracy bonus on punishes. Main weapon: zombie axe or Zamorakian hasta → abyssal bludgeon, elder maul or Dharok's greataxe → soulreaper axe or scythe of vitur set to crush → Inquisitor's mace (best only with a 4-tick punish cycle). Dedicated punish weapons, best first: Dharok's greataxe (full set, under 50 hitpoints), soulreaper axe above 3 stacks, elder maul, colossal blade, scythe or a godsword, zombie axe or barrelchest anchor. Armour: elite void → fighter torso, Bandos or blood moon → Torva or Dharok's → Inquisitor's. Neck: fury → torture → rancour or blood fury. Cape: mixed hide → fire or mythical → infernal. Defender: dragon → avernic. Gloves: void or barrows → ferocious. Boots: dragon → aranea → primordial → avernic treads. Ring: berserker (i) → ultor. Special attacks: dragon warhammer → elder maul → crimson kisten. A slayer helmet (i) beats all helms when you are on a Maggot King task from Konar, Nieve or Duradel (3,675 Slayer XP per kill, brimstone keys on Konar tasks).
